As children grow to adulthood they may require support, particularly if they have received support in childhood. Depending on the services provided, transition support will begin from the age of 14 to 25 years, as they receive adult disability services, Probation Services or Leaving care support.

This Serious Case Review was published in October 2017 by Bristol Safeguarding Adults' Board. It is about the tragic murder of a young woman called Melissa. It has learning about transitions between children's and adults' services, especially for children who were looked after.
